Submitting a Protocol

Protocol Application:

All protocols should be submitted ELECTRONICALLY ONLY to the GCRC office, Room 12S-136 in the hospital or emailed to Diane.Ruenes@Stonybrook.edu .

All submissions must include:

• GAC Protocol Cover Sheet
• Protocol Submission Form
• Protocol in NIH Format
• Data and Safety Monitoring Plan Template (DSMP)
• Biosketches for ALL Investigators listed in the Protocol in NIH format

To download the forms, please visit HERE.

If IRB approval has been received, submit 1 copy of each of the following:

• IRB approval letter.
• Stamped consent form(s).
• 1 copy of the IRB application.

Protocols can be reviewed simultaneously by the IRB and the GCRC General Advisory Committee(GAC), but the IRB approval must be granted for the the study to begin.
